solidify threading and make such optional

These threading changes pass with both asan and tsan enabled now.  New
cmdline args are born:

  -t [ --thread ]                  use a separate thread for bam io
  -b [ --buffer-size ] arg (=4096) size of the ring buffer between io and
                                   processing thread (# bam records).
                                   ignored without -t/--thread
