Commit Message

Remove `Question#is_mandatory`

- Removed from the database because it is always an exact duplicate of `required == ‘required’` which just added confusion when understanding the code.
- Renamed `is_mandatory?` to `mandatory?` on the model to be more Ruby idiomatic.
