Patch the mongo connection pool

ismaster checks occuring on behalf of incoming requests
were escaping. By binding listeners on the connection
pool event emitter to the current cls context, we can
correctly attribute the work that they do.

There were some substantial changes required to the
instrumentation structure to make it possible to patch
individual files within a module. This same structure will
make it possible to patch multiple versions of a single
module.
