units parse iso8601 durations, py3 shebangs, remove transform_value

first pass implementation of a sub-parser for iso8601 durations a
separate set of unit prefixes/suffixes is used since the letters
used are specific to iso8601 and are not generalizable

transform value is replaced by BIND f λv.RETURN g(v) which simplifies
the compositional language for the combinators

note that the units for postnatal-day have been corrected to be
postnatal-days
