streamer: Do JSON serialisation in process_message

This commit moves responsibility for:

- serialisation of outgoing messages
- the act of sending to the websocket

out of the two message handlers (previously broadcast_annotation_message
and broadcast_session_change_message, now handle_annotation_event and
handle_user_event) and into process_message. This simplifies testing the
message handlers substantially and reduces duplication of code.
