Change all case conversions to apply acronyms configured on an instance

All the acronyms configured on String::Case instance will be applied to every
case conversion. These acronyms can be overridden on a method basis by
providing an acronyms keyword parameter. The parsing of acronyms has
also improved to account for acronyms combined together with other words
or acronyms.
