Parse math shifts in horizontal lists

This allows switching into math mode when we see a math shift, parsing
a math list, converting that math list into a horizontal list, and
appending that horizontal list onto the end of the current list. The
rules for display math (`$$`) are significantly more complicated, so
I'll figure those out later. To allow returning multiple horizontal
list elements, I changed the return value of
`parse_horizontal_list_elem()` to be a new enum type. I also ensured
that vertical mode will correctly drop into horizontal mode when a
math shift is encountered.
