Update prometheus readme and grafana templates/images (#333)

* Update prometheus readme and grafana templates/images
- modbcounts + grafana graphs (along with templates).
- automatic service discovery info to scrape from opflex ports with recommended prom server configs.
- Update opflex peer uid to avoid clash with server graph
- Remove active ep count from EP graph and add alert config info in template
- Reference to updated port allocations

* Fix typo in AgentCounts.json and also change font of alert text in prometheus.md
