Rewrite insert and delete content element using new Batch

The new Batch object allows expresing the required transformations
more concisley. Some minor test changes are required:

* Since all changes are now only applied after the request has
  finished, we need to add fake server responses in some cases to
  observe the change.

* Before some of the changes were applied optimistically. Now changes
  are made only after the request finishes. We remove some
  expectations that capured this in between state.

* Batch items with delete flag now always come at the end. This
  prevents gaps in the server generated sequence of positions.
