Add JSDOC documentation for classes and modules (#83).  Refs #78.

* Initial jsdoc stuff
* Use jsdoc.json for template
* Fix formatRequest test
* Update formatRequest params
* Ignore comments and template string length in eslint
* Change how clientConnected and clientDisconnected work
* Remove 'close' listener
* Include README in docs
* Change wsclient import name
* Rename handle functions to prefix with 'got'
* Add helperfunction to get class methods starting with 'handle' (for future implementation)

* Change type and default of jsonrpc version to use numbers (i.e. 1 and 2) for cli tool

* Use process.stdout/stderr.write to print to terminal
* Move sendNotification to base factory
* Increase code duplication mass value in codeclimeate.yml
