Treat unfinished tests as failures

This is potentially a breaking change for some cases.  However, I'd
argue that those cases are only working by accident, and several people
have complained that the prior behavior is a bug.

If your test stops working because of this, then it means you need to
add either a `t.end()` or `t.plan()` somewhere, to ensure that all your
tests have been run before ending.
