Replace exometer with prometheus (#158)

These commits replaces exometer/exometer_core libraries with prometheus.erl
to collect RADIUS metrics.

Besides replacing of metrics library, there are following changes were done
related to metrics:

  * Metrics names are updated according to prometheus metrics naming. Previously
exometer metrics were represented as a list of atoms, like for example:

[request,malformed,server,$NAME,$IP,$PORT,total,undefined,undefined,counter]

Now the same metric named: eradius_malformed_requests_total, and variadic parts
like $NAME, $IP and so on made as prometheus labels.

  * NAS and Server metrics were combined into one. Previously these metrics
were splitted per NAS and Server but as the current implementation provides
prometheus labels - the metrics were merged.

All the counters were implemented on top of eradius_counter ets based
storage and should be accessible via API. Histogram metrics were implemeneted
based on prometheus_histogram collector as it is the easiest way to do this and
we don't need to manage buckets and other histogram related things in eradius.
