qa: run psalm autofixes

Ran `psalm --alter --issues=MissingReturnType,MissingClosureReturnType,InvalidReturnType`, which produced the first round of fixes found in this patch.
From there, updated some autogenerated annotations, primarily in unit test data providers, to be more generic.
Finally, ran tests and coding standards, and revised accordingly.
