Tweaks to MFA Recent Implementation (#599)

* Remove username from the password reset email per Ezra's recommendation.

* Fix typo in default value of AXES_ONLY_USER_FAILURES setting (should have been True).
Other minor tweaks to this documentation.

* Additionnal tweaks to MFA_REQUIRED.
Add settings to control if Admins and SSO users are required to set up MFA when required.

* Add tests for MFA_REQUIRED settings.

* Add support for ISO 8601 duration formatted strings for the AXES_COOLOFF_TIME setting.

* Clean up tests.
