Add TOTP based two factor authentication (#87)

This one is a bit hairy as the 2fa flow includes both TOTP (new) and U2F (existing).
Adding in the TOTP based authentication required re-working the entire existing
authentication flow to ensure that both would continue to work as expected.

To enable TOTP based authentication, another environment based configuration
variable is now required: TOTP_ENCRYPTION_KEY. This key is used to encrypt the
TOTP secrets in the database to reduce the risks of a single breach.

In the process of enabling TOTP, U2F broke and required a bit of re-working that as
well, which resulted in resolving another issue with U2F devices regarding naming
them (#42).
