Use formSubmissionField as obs.key

- Each QuestionnaireResponse.Item uses the obs.fieldCode as the Item.linkId in the EventConverter, this fieldCode is not always available. This has been substituted with obs.formSubmissionField
- For cases where both obs.fieldsCode & obs.formSubmissionField are absent, the obs will be skipped
