Overview of the changes:

- Renamed Channel to ClientCallFactory
- Created a new Channel interface containing the public methods from ChannelImpl
- Modified ClientInterceptor to be a factory for ClientCallFactory instances.
- Modified the compiler to use ClientCallFactory rather than Channel.

This takes several steps towards addressing #545.
