fix failing to run teardown if no setup

- refactor async bits in `Mocha#run`
- add `Runner#runAsync()` which just wraps `Runner#run` in a `Promise`
- rename tests (`global-setup-teardown` => `global-fixtures`)
- remove use of `rimraf` in `test/integration/helpers` since we already have `fs-extra`
- reorganize `test/unit/runner.spec.js`. the only thing added were the tests for `Runner#runAsync`
- `npm audit` upgrades
- revert whatever I did to `rollup.config.js`
