Fix mac failing to launch with some wxPython versions (#3056)

* BF: only make tools once (wx.Realize() on some wx versions not safe twice)

* BF: only make tools once (wx.Realize() on some wx versions not safe twice)

We mustn't call makeTools twice in quick succession (i.e. during app load)
on Mac even though it CAN be called again later (during theme switch).

So that we don't have to call makeTools twice we needed to avoid setting
toolbar buttons to Enable/Disable before they're created (easy)

With that issue solved, the call to ClearTools() became OK again which
simplifies that code too!
