* Modify log validation for polyhedral in mixed unstructured topology

* * Revised the top-level documentation associated with polygonal/polyhedral topology specification.

* Remove commented-out workarounds

* Name temp node for temporary data

* Cleanup indentation

* Remove size and offset variable requirements for polyhedral topology

* Refactor polygonal/polyhedral verify

* Remove optional offset variable from unstructured non-poly verify()
