Fix for signature version 4 signer signed headers.

The previous implementation relied on the http request headers
having a case-insensitive hash of header keys (all downcased).
To make the v4 signer more portable, the `#signed_headers`
implementation now downcases all header keys so that it may
work with a vanilla ruby hash.
