Remove v1 hostnames support (#41)

* Remove v1 hostnames support

This change removes the --name-version flag and the corresponding
conditional logic to support both v1 and v2 hostnames.

* Merge branch 'master' into remove-v1

* Fix typo.

* Add go.mod/go.sum

Apparently Travis downloads a broken version of stretch/testify which
breaks the bmctool build. Using go mod in this project should fix that.

* Flags -> flag.

* Downgrade testify to v1.4.1.

* Use go1.13, go mod and upgrade testify to v1.6.1 again.

* Flags should *really* be "flag".
