Support schema.items in SwagResponseSchema and other refactors #125

Annotation/SwagResponseSchema
- Added `schemaItems`
- Add/update unit tests in SwagResponseSchemaTest

OpenApi/Content
- Removed properties from that are meant for OpenApi/Schema
- Support serializing `schema` property from object or string
- `mimeType` property can be null

OpenApi/Schema
- Add `refEntity` property and support its serialization
- getFormat() can return null now

Operation/OperationResponse
- Refactored assignDefaultResponses() to support changes
- Add/update unit tests in OperationResponseTest
- Added schemaItems action to test_app/src/Controller/EmployeesController to support integration testing

Misc:
- Updated ReadMe for SwagResponseSchema
- Updated comments in SwagResponseSchema with examples and such
