Add shift notes modal in responders console

Responders need to be able to view if there are any shift notes when
reaching out to responders, and this allows them to view those in a
modal dialog if there are any notes for the day.

Finishes fixing Issue #207

Issue #207: Add a Shift Notes link (https://www.dcsops.org/scheduler/shift_notes)
            to the DAT Scheduling page
