Big cleanup

- Add missing symbol comments.
- Level names and constants now have type-aliases.
  - This is not backwards compatible. We created a v2 release.

- Stop exporting symbols that do not need to be exported.
- Eliminated error-definitions. None of them need to be checked
  anywhere.

- log() now panics on errors rather than returning. We ran into some
  issues that didn't crash as expected when this wasn't checked on
  return.

- log() now only returns an error when it's an error level.
