Handle EOF more alike the other grammar rules

- Make the codegen template generate the method for EOF together
  with the other grammar rules.
- Moreover, ensure that EOF is added only once to the internal
  graph representation, even in the case of non-combined (i.e.,
  separate lexer and parser) grammars.
