fix remaining type/export issues

- set types prop in package.json
- ignore src in npm tarball
- finagle the default export (with properties) of the package "main"
(mostly backwards compatible. the only thing missing is the callable
aspect of the main export. const { create } = require('faktory-worker')
could be used, though)
- consolidated utility functions into one file
- converted all exports to named, no defaults
