combine:  Add include/exclude layer filtering capabilities

- Add "--include" and "--exclude" arguments to ksconf combine to allow control
  of the selection of which filters to combine.  Individual filters can be
  named explicitly or matched via a simple glob pattern.
- Unit test added for new LayerFilter() class, covers 6 scenarios.
- Implemented layer filter in the LayerRootBase class as a one-time
  apply_filter() operation.  This is quite simple, and fast, but it isn't very
  flexible and once the layers are removed (from memory) we'd have to reload the
  class entirely to restore that information.  Good enough for now.
