Revisit logic for agent connection tracking

Rather than rely on gRPC connections status callbacks directly calling into the routing service, introduce a new intermediate service which tracks individual connections.
AgentConnectionTrackingService will timeout connections if heartbeats are not received.
It will also de-duplicate agents coming on over different connections.
