Port to CategoricalArrays 0.8 (#2188)

Replace `CategoricalString` with `CategoricalValue{String}` and
stop using `CategoricalArrays.index`. This simplifies the code a bit.
Also drop some specializations which are equivalent to optimized methods
defined in CategoricalArrays.
