Rework of the CSHAKE API

This commit spiraled out of control.  The original intention was to just remove
setCSHAKE parameters and move the logic into the SHA3 constructor.  I then moved
packedConcat into common.ts from sha3.ts (might use it elsewhere).  Integrating the
new SHA3/CSHAKE constructor into sha.ts then caused a rework of the types.  This
rework exposed an issue with rollup.
