Introduce grants-based role whitelisting

Grants-based or Permission Derived whitelists is a convenient way to
configure whitelisting, without having to configure too much details.

The following statement:
cqlsh> ALTER ROLE kalle WITH OPTIONS = { 'GRANT AUDIT WHITELIST FOR MODIFY' : 'grants/data' };

Will whitelist the "kalle"-user on MODIFY operations to all
keyspaces/tables he has permissions to modify.

If he tries to modify a table where he lacks permission, not only will
the operation be unauthorized, it will also be audit logged.
