Harden ConnectionManager with 2 new exceptions

This commit introduces IllegalConnectionNameException and
NoSuchConnectionException which are used with addConnection and the
magic __call function

These prevent connections being added with the name APPLY_TO_ALL and
__call from attempting to index a connection that doesn't exist
