Correctly detect Python source file encoding

Using open() on a Python source file will not work if the system locale
doesn't match the source file encoding.  Let's use tokenize.open()
instead which will look for # -*- coding: ... -*- declarations and such.

Closes #24.

Also drop very outdated Python 2.3 (!) code examples from the module
docstring.
