Doc: recommend Postgres 12 and PostGIS 3

I have been using and testing this for a while without problems and
could make use of some performance improvements due to compacter
indices, improved parallel queries and better CTE handling.

This requires an update of replication instructions, which now default
to Postgres 12 as well.
