Merge pull request #234 from PDLPorters/fix_docsys

Fix documentation system: restructure the documentation database pdldoc.db to allow more than one function with the same name to be present in the documentation database.  Adds module information to 'apropos', as well as some commonly-used shorthands for module names.  Correctly handles multiple functions that share documentation.

This commit also adds a new dependency to PDL, Term::ReadKey, to more robustly handle screen width calculations in the perldl shell.
