ENH: Add shims.min.js

Add shims.min.js at the top of every bundle.

MAINT: Rename polyfills.min.js to polyfills-es5.min.js

DEP: Remove some polyfills and move others to shims.min.js

Remove Array.from, Number.isNaN, DOMTokenList#forEach,
DOMTokenList.toggle (2nd argument), Element#matches.

Move Object.assign, Object.values, DOMTokenList#replace to shims.min.js

MAINT: Require core.js directly, instead adding it to the wrapper

MAINT: Move metadata to the end of the bundle
