Adds item support to CheckoutPricing#adjustment

CheckoutPricing#adjustment will now accept an itemCode, which Recurly.js
will use to retrieve price information when constructing an adjustment.
Overrides are accepted for all properties.

An itemCode may only be used when constructing a new adjustment. When
modifying an existing adjustment, passing an itemCode will throw an
error.

Bonus: Improves documentation of CheckoutPricing class
