Wrap all errors in `ShareDBError`

This change drops our dependence on `make-error`, and instead implements
our own simple `ShareDBError` class, which extends `Error`.

We also wrap all anonymous error objects in this `ShareDBError`, so that
its type can be checked by consumers.
