Disallow user info in URIs by default, fix referrer and HTTP/2 authority

The user info component is deprecated, so we disallow it by default.

The 'referer' header was set but immediately removed again due to a bug.

If the user info component was present, it was sent in the HTTP/2
authority and therefore resulted in bad requests.

Closes #211.
