allTests now pulls in all test source code via #include

The intent behind allTests was to force the templated classes to be in a
 single binary, so that a unified coverage metric could be generated.
 This was not successful, since the object is still declared in each
 object file. This version of allTests pulls in the source code,
 forcing it into a single object file at copmile time.
