Fix wrong tx_domain_dist flag with default Psychovisual

- The encoder says tx_domain_distortion = true, which is wrong because
  the default tune::Psychovisual disable use of tx_domain_distortion.
- Remove all "TX domain distortion" comments and messages.
- Set use_tx_domain_distortion false if tune::Psychovisual is turned on,
  which is default.
