* Fix invalid Heroku app in client command

* Do not hardcode Heroku app when determining admin credentials

Pass the correct app to `heroku config` when determining the admin
credentials. The app name is the subdomain used in the request URL.

* Restore test coverage for client command

* Update CHANGELOG.md
