Use localhost in DATABASE_URL outside of Docker Compose (#35)

* [docker] Specify DATABASE_URL in Compose file

Add DATABASE_URL to docker-compose.yml, using `postgres` as the hostname. In
Docker Compose, the database server runs in a separate container.

This definition overrides the one specified in .env.sample, which assumes the
database server runs on localhost.

* [.env.sample] Use localhost in DATABASE_URL

Assume that the database server is running on localhost during development.

* Update CHANGELOG.md
