monotime: avoid overflow when converting between coarse and ms on Apple

This fix makes sure that the token bucket rate_per_sec_to_rate_per_step()
can not overflow with large rates on iOS, and any other platform where:
  monotime.numerator^2 / monotime.denominator > 512

It also avoids overflow for any other callers that convert values of:
  msec > 2^64 / (10^6 * monotime.denominator)
  coarse_stamp > 2^64 / (monotime.numerator^2 / monotime.denominator)

(These bounds are approximate: the code does exact calculations.)

Fixes bug 31482; bugfix on 0.3.4.1-alpha.
