Deprecate the ServiceAccount class

This decouples credentials from retry policies. Instead of the ServiceAccount class, we now have a FaultTolerance class that you can pass to the Configuration class.

While here, remove the requirement in Kerberos/SSPI auth mode for a dummy Credentials object.
