doc: Adjust all guides to use Helm templating

* Convert all guides to either use:

  1. `quick-install.yaml` when no customization is needed
  2. A `helm template` command line for all more advanced configurations

* Switch to a CRD-based default backing to no longer require an external etcd
  or etcd-operator driven etcd cluster by default.

* Extend the upgrade guide to instruct users to extend the ConfigMap as
  required for 1.6 upgrades

* Remove obsolete upgrade notes for 1.5, 1.4, and 1.3
