De-duplicate indexes [ECR-3359] (#1039)

De-duplicate Java indexes to overcome the current
limitation of MerkleDB which permits only a single
'open' index with a certain address in a given fork.

This PRs restores the possibility to create them
whenever convenient — the first request will open
an index, all subsequent will reuse the instance.
