patch: Detail failures via exception chains

Instead of bluntly raising an exception with the whole of patch's
contents when parsing fails in the patch module, raise exceptions
explaining the failure, and chain (PEP 3134) them to the patch's
location, so both the source of the error and its details could be
communicated.
