ir: use Module.WriteTo to implement Module.String

Also move formatted I/O writer to helper.go and add
documentation.

Fix signature of WriteTo to conform with
io.WriterTo interface. In particular, the
return type of `n` has been updated to `int64`.

Thanks to @dannypsnl for providing the initial implementation.
